Replacement of Birth Certificates (BC) lacking ID photos
Birth certificates issued without ID photos, for Iranian children, is to be replaced by Birth certificates that include ID photos, once the child has reached the age of 15.`
The relevant application documents will be transferred to the respective registration authorities in Iran and upon the issuance of BC, the ID document will be delivered to the applicant.
- A filled out copy of application form [no. 211] for replacement of BC. (for children under 18, a declaration letter must be written in this regard and undersigned, by the child’s father)
- Original copy as well as a photocopy of all pagesof the applicant’s Iranian Birth Certificate(Shenasname).
- Original copy as well as a photocopy of any other valid ID document
- Four 4x6cm (Including a full face, front view, open eyes, without a hat, glasses or neck scarves with the exception of headscarves(hijab for women)). [The individual’s personal details must be inscribed on the back of every piece of photo]
*Payment of the application fee at the consular section of the Embassy.
*A photocopy of applicant’s Dutch Residence permit or Identity Card (Identiteitskaart ).
